54 yo black diabetic male presents to wound clinic with recurrent bullous lesions to bilateral lower extremities. Lesions spontaneously occur, rupture, then crust over. Denies pain; hx of paraplegia. I suspect this may be bullous diabeticorum. Treated him with a course of clindamycin, prednisone, and silvadene. Also referred to dermatology. Any ideas what this may be? Concerned because lesions occur over saphenous vein. Maybe vasculitis?

Bullous progression of stasis ulcers bilaterally in a patient with thrombocytopenia and DM2. These lesions began arising spontaneously during his hospital stay, confined to the lower extremities. The largest of these was on the opposite leg, roughly the size of a tangerine fruit.
