AP radiograph of the pelvis shows multiple osteochondromas arising from the pelvis. Many of the lesions arising from the pelvis are pedunculated while the lesions of the femurs are more sessile. Patients with multiple hereditary exostoses are at higher risk of developing chondrosarcoma. #MSKMonday

Cervical Fibroids Most of the leiomyomas are situated in the body of uterus. Fibroids arising from cervix are rare tumours accounting for 2% of all fibroids. A central cervical fibroid is usually either interstitial or sub mucous in origin and usually arises from supravaginal portion of the cervix so that it expands the cervix equally in all directions and can displace uterine vessels & ureters. On laparotomy it can be recognized at once, as it fills pelvis, with uterus on top of tumour like "The Lantern on the top of St. Pauls". AP (image 1) and detail AP radiograph (image 2) and axial T1 MRI after contrast (image 3) show a large Ewing sarcoma of the mid femur. The mass has faint speckled calcification (arrow) and on the MRI is shown to arise from the posterior surface of the femur. Ewing sarcoma is the second most common malignant pediatric bone tumor. It usually arises from the diaphysis of long bones. The overall 10-year survival for patients with Ewing sarcoma is 60% if the tumor is localized. Survival drops to 30% if metastatic disease is present. 70 yo WF with 20y hx HTN lost 21 lbs on a semaglutide but still BMI over 30, shows drop in BP when standing, stabilizing at 102-116 systolic, 65-75 diastolic. Discontinued all HTN meds x 30 days and no episodes of HTN. Could a small weight loss like this cause a drop to normal BP?
