Axial T2-weighted (image 1) and FLAIR (image 2) MRI focusing on the posterior fossa in a patient with new-onset vision loss treated for medulloblastoma with surgery and radiation shows enlargement and a circular area of increased signal within the olivary nucleus consistent with hypertrophic olivary degeneration. Hypertrophic olivary degeneration is associated with palatal tremors, ataxia, muscle weakness, and visual and gait impairment. #NeuroWednesday
