Elderly pt p/w Nail discoloration x several months. Dx and Tx, if you please?


37 year old patient. Was diagnosed auto-immune hepatitis in the past but now liver enzymes are normal. Patient now perfectly healthy with no dm, not hypertensive. Complaining of yellowish-greenish (more green than yellow) discoloration nails and also horizontal ridges on some nails as seen in picture above. Are these beau lines with #Onychomycosis / #pseudomonas infection?


72 year old lady with cognitive decline and anxiety. Reports that her daughter stepped on her foot 5 weeks prior and the nail was lifted and bruised. There was a sweet/ fermented smell coming from the nail so I removed the nail plate to investigate. We have not taken cultures but are treating based on clinical signs. What bacteria is this and what treatment methods would you use?
