Pt is 1 month old, born full term, however admitted to the NICU due to respiratory distress. Pt noted to be hypo cephalic, has B hip dysphasia, rocker bottom feet, and concern of chromosomal abnormality due to parental consanguinity. Pt also presents with persistent, ‘thumb-in-fist’ positioning, and the RNs were complaining of odor, and difficulty with cleaning pts palms, so thumb loop splint was provided. PROM is WNLs at all the hand joints, except for B thumb MP joints, which have flexion contractures. Recommended to alternate splint from right to left had at each care time, which is ~ q3h

Rocker-bottom foot, or congenital vertical talus, is a rare deformity of the foot characterized by a prominent calcaneus and a convex plantar surface. Its presence on obstetric ultrasound is sometimes considered a soft sign for aneuploidy. This newborn had trisomy 18, or Edwards syndrome, a condition associated with congenital malformations including microcephaly, microphthalmia, and distinctively clenched fingers. The majority of cases diagnosed prenatally die in utero or shortly after. Those who survive have severe intellectual disability. Image credit: @IBercholcMD.
