2cm brown patch with irregular borders, asymmetry, and multiple shades of brown. Very concerning for melanoma. A biopsy was performed with a #15 blade. If you see a lesion concerning for melanoma, excisional biopsy or deep shave is required for path. Never take a partial sampling.
Similar cases
patient presented for a mole check with concern about a mile present for 1-2 years. Family history was significant for melanoma in a first degree relative. Shave biopsy was consistent with melanoma and a breslow depth of 0.48mm without other high risk features. Wide excision was subsequently performed.
27 yo healthy male presented to the family medicine clinic with new skin lesion that developed on his 4th digit of his right hand. He first noticed this lesion around one month prior. Denies any trauma to the area. He originally thought it was “dirt”, but it did not come off with cleaning. The lesion is not painful and does not itch. On exam the lesion is flat with the skin, and appears as flat brown macules. What could this be? DDX? Attending in the clinic thought maybe solar lentigo vs new Nevis, but would need biopsy to rule out malignancy such as possible superficial spreading melanoma. Patient was sent to Dermatology for further evaluation.

found on a 30yo male. He remembers having it for a long time but cant recall if he had it as a child. Looks like a congenital nevus... his wife is concerned about possible melanoma. Referral to derm.
A 52-year-old man presented with a 20-minute history of chest pain radiating to the left shoulder and neck, associated with dyspnea and diaphoresis. He had a history of hypertension, long-term smoking, and a sedentary lifestyle. On physical examination, bibasilar crackles were noted on lung auscultation. An electrocardiogram (ECG) was performed in the primary care setting, and the patient was promptly referred to the emergency department.
A 28-year-old woman presented with a 3-day history of intense pruritus over the lower back. She reported no recent travel or exposure to new environments. She cares for a small kennel with approximately eight dogs rescued from the streets. Physical examination revealed multiple small vesicles, some clustered and others scattered, predominantly involving the lumbar region and the left gluteal area.
A 4-month-old male infant presented with skin lesions localized to the chin for the past 3 days. Physical examination revealed multiple small pustules with surrounding inflammatory signs on the chin, along with a few scattered papules on the chest.
patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family, are there any features in this ekg that would warrant further work-up or is this just a pediatric ekg?
