Case Overview

This is a case of a 22 y/o, G1P0, pregnancy uterine, at 28 2/7 weeks AOG who came in due to preterm labor. Patient delivered vaginally to a live preterm baby girl and an abortus (shown above) that was comparably smaller than the other twin. Multifetal pregnancy that is MoDi was unknown to the mother due to lack of prenatal care. Probably a case of twin to twin transfusion syndrome.

Description: A bit of a sensitive case but quite rare. A 26 year old woman came into labour Ward P1 G3, 29 week gestation with twins. She was unbooked and presented today. Poor obstetric history, previous fresh stillbirth and miscarriage. Of note was that there was cord presentation. So she was rushed into theatre with 6cm dilated cervix and a C-Section was performed. Conjoined twins, weight 1.8 kg, they had other malformations like an imperforate anus, ambiguous genitalia and fused arm/hand. Unfortunately, they passed away. Mother received counseling and psychological support from the hospital.

Description: Diamnionic dichorionic twin full term placenta showing thick meconium in only one fetal sac.

Description: Fetus papyraceus. The fetus died in the 16th week in an otherwise healthy twin-pregnancy. The healthy baby was carried full-term.

Description: Velamentous cord insertion of the placenta. Needed to use forceps because fetal heart tones dropped with pushing, likely due to compromised blood supply from placenta.
