case of a Live baby boy, delivered term to a G2P2 mother (no maternal illness) via CS secondary to Nonreassuring fetal heart rate pattern. Birthweight: 446kgs, AS:8,9. What causes true cord knot?

This is a case of a 22 y/o, G1P0, pregnancy uterine, at 28 2/7 weeks AOG who came in due to preterm labor. Patient delivered vaginally to a live preterm baby girl and an abortus (shown above) that was comparably smaller than the other twin. Multifetal pregnancy that is MoDi was unknown to the mother due to lack of prenatal care. Probably a case of twin to twin transfusion syndrome.
