This pt presents with BCVA OD HM OS 20/60. S/p multiple RD and VMT OD 10 years ago resulting in pre-phthisis Bulbi. OCT reveals vitreomacular traction OS. Referred to retina for evaluation since it's his only functional eye. Retina chose to observe secondary to patient hesitation to go forward with more eye surgery on his only functional eye. 79 year old Caucasian male presented with a referral from the primary care OD for a ONH lesion. He complains of mild decreased vision in OD which was found to be early PSC.
VA 20/25 OD 20/20 OS. chair skills normal. anterior segment findings normal with age related NS OU and trace PSC OD. DFE revealed white elevated lesion nasal to the ONH with RPE disruption nasal and inferior to the lesion.

OCT raster in image 2 shows elevated lesion with a lot of hyperreflective material approximately at the level of the OPL.
Cirrus OCT advanced visualization tool in image 3 shows the RPE disruption and reverse shadowing in the top right picture.
I'm not sure what this is and it may be changing so I'm referring to Retina.
