Fever and rash. What is your diagnosis?

22 years girl with normal cbc. She has pharyngitis only erythmatous. Fever from three days ago. Her brother has exudative pharyngitis without rash. He has 8 percent atypical lymph. Please help for diagnosis. No lymphadenopathy.
