The patient is a 64 y/o female presenting for c/o Right upper extremity pain for 1 week. The patient describes left upper extremity pain, intermittent sharp pain starting at scapula radiating down to fingertips. Pain is severe, 8/10 in severity. The patient does not have symptoms of bruising, swelling, redness, warmth, numbness, tingling, weakness. The patient did not have a loss of consciousness, no vomiting, no dizziness present, no blurry vision.

Review of Systems: denies chills/sweats, denies shortness of breath, denies neck pain/stiffness, denies lightheadedness

Physical Exam:

Constitutional: No acute distress

HEENT: Mucous membranes moist

CV: Regular rate and rhythm

Respiratory: Lungs clear without crackles,

Extremities: Non-tender. No edema. no pain to palpation, limited ROM b/l upper extremity. Left hand 2/5, R hand 3/5

Back: No tenderness

Neuro: No gross motor deficits,

\ \ Pt is a 71y/o F presenting for a phone progress visit.\ #COVID-19 screening - Pt reports fatigue from doing laundry and getting ready for the nurse -Pt states son was exposed to two people at work that had COVID, and he came by her home to drop something off. States he didn't come inside. -Her son was sent home from work for three weeks. -Pt called and told nurse that she was around her son who was exposed to people with COVID. -Pt says it was okay if the nurse didn't come for a week, so they sent her what she needed to care for her wounds.\ #Ulcer of L calf -Pt reports the last time the nurse saw her wounds she said they were looking better. -Pt states taking good care of her wounds. -Reports the nurse is wrapping them differently with the gauze instead of using tape -Pt states nurse is coming 3x/week after not coming for 1 week d/t pt's son's COVID exposure -States inflammation decreased since the nurse has been wrapping it in different ways -Pt states pain in her leg d/t ulcer.
