5 day old boy with cyanosis since birth was found to be TGA on evaluation. X ray is pretty typical of 'egg on end' appearance. ECG shows Right axis deviation and right ventricular predominance (thumb rule most cyanotic congenital heart diseases have right axis deviation except tricuspid atresia). Child did not have VSD or PDA - underwent Balloon Atrial Septostomy immediately as saturations were only 60%.
