Varicocele
Ultrasound imaging of the scrotum shows a varicocele characterized by multiple large, serpiginous vessels. A varicocele represents a dilated pampiniform venous plexus within the spermatic cord. Patients may present with a scrotal mass, infertility, or testicular atrophy. On physical examination, a varicocele is described as feeling like a 'bag of worms.' On imaging, the veins are dilated more than 2 to 3 mm in diameter with increased flow during the Valsalva maneuver.

An image of the kidney after a retrograde pyelogram shows a filling defect (arrow) within the markedly dilated lower pole renal collecting system. The patient was diagnosed as megacalycosis, a nonobstructive dilation of the renal calyces. Megacalycosis typically affects one kidney. While this condition is associated with marked dilation of the calyces, there is often no dilation of the renal pelvis or ureter. Megacalycosis is associated with stone formation.

Frontal (image 1) and lateral (image 2) chest radiograph shows cardiomegaly and mild hazy opacity in the upper lungs. Axial contrast-enhanced CT (image 3) confirms that the heart is enlarged due to dilated cardiomyopathy. Dilated cardiomyopathy is the most common type of cardiomyopathy. It usually occurs in adults between their third and fifth decade of life. Up to one-third of patients with dilated cardiomyopathy have a genetic source for disease. Other potential causes of dilated cardiomyopathy include coronary artery disease, diabetes, viral infection, drugs, and alcohol abuse.
Axial T2-weighted images show multiple dilated perivascular spaces within the globus pallidus (arrow, image 1), thalamus (arrowhead, image 1), and midbrain (arrow, image 2). What is your differential diagnosis for multiple dilated perivascular spaces? #FridayQuizDay #FridayQuiz
