Here is a case of an invasive squamous cell carcinoma of the larynx, specifically the right aryepiglottic fold. I’ve marked some helpful anatomical landmarks on the first picture to help orient yourself — we are viewing the larynx from the posterior aspect (as if the patient were facing away from us). You can see the yellow tumor at the right AE fold which extends inferior and medial, across the midline. The black suture on the right was marked by the surgeon as the closest mucosal margin at the right AE fold. In the 2nd pic, I show a cross section of the tumor (present at the top middle part of the picture) in relationship to maybe important structures/spaces. Evaluation of the cricoid cartilage, thyroid cartilage, pre-epiglottic space, and the hyoid bone (among several others!) are crucial in determining stage. With just a handful of the needed anatomical terms and locations mentioned in this case, you can see how these cases can be quite challenging from both an anatomic pathology and a surgical point of view.
