1st strip is in response to the first .5 of atropine. Pt responded briefly, but the junctional rhythm was refractory and the pt then became hypotensive. Fast patches were placed in prep for TCP, but a second .5 of atropine was given. The pt then converted. His skin pinked up and he became normotensive.

19 Yom. CC was syncopable episode. Pt a/ox4 gcs=15 upon ems arrival. PT girlfriend states pt had approximately 30 second of unresponsive after complaint of feeling dizzy. PT initially refused all services even an assessment. Pt was able to be talked into full assessment but did not want transport due to not having insurance. Pt stated he thought he was hypoglycemic due to not eating breakfast and states 5 previous episodes in the last 3 months. After repeated request pt agreed to transport. Pt has no medical hx. Takes no medication. Denies drug or alcohol use. Initial assessment : BP128/76. 1st picture is initial 12 lead, ss slightly pale, cool, clightly clammy at core. respiration 14 non labored, spo2 98%. BGL 64. Initial treatment 16g in left ac. en route to hospital about 2 min from er pt reports "I don't feel so good" has another syncopable episode lasting approximately 30 seconds ( 2nd picture: snapshot of 4 lead during syncopable) 3rd picture is 12 lead during syncopable. BP 58/P, ss:extremely pale, cool, diapheritic. Pt give rapid fluid bolus, .5mg atropine iv, 14g iv in right ac with 2nd fluid bolus. BP improved to 112/68, 4 lead ecg st at 120. Per er staff pt labs where "completely normal" pt became Brady again at hospital, was given enough more atropine , pt did not want a bill so he snuck out of er. C/t 89 yo female c/o intermittent diarrhoea and significant weakness for 2/52. Pt has been bed bound for 1/52 due to weakness. HR on arrival 30bpm and BP 80/40. All other obs wnl. Nil pain, GCS 15. PMH: AF, x2 CVA's 2016, x3 MI's over last 10 years, CABG 2014, hx of SVT and ablation therapy in 2015, HTN. Meds: Bisoprolol, Digoxin, Doxazosin, Atorvastatin and warfarin. 12-lead shows significant bradycardia with bigeminy at 34 bpm. IV access gained and x2 doses Atropine administered (total 1.2mg). 1st ECG (pre Atropine), 2nd ECG (post 1st dose Atropine), 3rd ECG (post 2nd dose Atropine). Atropine only increased HR to about 45-50bpm but BP improved to 160/80. Pt conveyed to nearest A&E. What is the rhythm?
