I reached back out to pathology who confirmed Martorell ulcer. They were unaware of this pathology and after further examination they noted thickening of the arteriolar walls and focal intravascular calcium deposit. His mri of adrenals was normal, now pending adrenal vein sampling to determine cause of his uncontrolled hypertension. His hypertension has been out of control for 10 years per him. The hypertension worsened more last year after losing 50 pounds. Wound now is almost healed with Regranex. First picture is today. Next is initial presentation 6 months ago, then after surgical resection and then necrosis of that area.

He had a history of hypertension, long-term smoking, and a sedentary lifestyle. On physical examination, bibasilar crackles were noted on lung auscultation. An electrocardiogram (ECG) was performed in the primary care setting, and the patient was promptly referred to the emergency department.
