Two sequential CT images of the pancreas (top and bottom) show a split of the pancreatic duct (going distal to proximal). The two pancreatic ducts (arrows on bottom image) surround the enhancing portal vein consistent with portal annular pancreas. Portal annular pancreas is typically an asymptomatic anomaly where the pancreas encircles the portal vein. Portal annular pancreas differs from annular pancreas where the pancreas encircles the duodenum. #TummyTuesday

Fluoroscopic image from an ERCP shows a bifurcated pancreatic duct within the pancreatic tail. In addition to this rare congenital anomaly, there are findings of chronic pancreatitis including dilation of the pancreatic duct and dilation and irregularity of the side branches.
On cross-sectional imaging, other findings of chronic pancreatitis include pancreatic atrophy, pancreatic calcifications, and abnormal parenchymal signal.

AP (image 1) and lateral (image 2) images from upper GI examination in a neonate with bilious emesis show contrast extending within the stomach and proximal duodenum (arrow). An enteric tube is coiled within the proximal duodenum. Neonatal duodenal obstruction can occur from duodenal atresia, duodenal web, annular pancreas, and malrotation with midgut volvulus. In this patient, annular pancreas was identified at surgery. Annular pancreas occurs when the pancreas parenchyma encircles the second portion of the duodenum. Patients may present in the neonatal period when the aperture of the annular pancreas is narrow.
