We all think we are plastic surgeons when it comes to suturing. This was my attempt on a 26 YR old make that hit a deer on his motorcycle and had the antler graze his face. Then the pavement grazed his face when he wrecked. He had exposed tendons on the left hand and right wrist. No fractures. He was discharged from ER with hand surgeon follow up.

This patient arrived to the ER early in the morning when a palate of cinder blocks slipped off the fork lift and landed on his left 2nd and 3rd fingers. Wasn't able to get the XR, but both distal phalanges were completely destroyed and would not be salvageable. We treated him with IM dilaudid and a vigorous digital block before he was emergently repaired by the orthopedic hand surgeon in the OR.

2 week old injuries suffered when driving 55 mph on a motorcycle and having a deer pop out in front of him. Kevlar vest, full face helmet, gloves and heavy boots. Skid marks only 15 feet long before he was on his faceshield and knees and hands and elbows doing 55 mph on the pavement. Had plenty of time to say to himself, "I am going really fast on my hands" and put more weight on knees and helmet until he stopped, unhurt except for road rash on elbows, palms, knees, ruined jacket, riding leather pants and boots ruined, and totally trashed helmet worn down to millimeters from his face.

Very simple case but I wanted to share the image. 30yo AA male got his ring finger stuck in a dirt bike chain when trying to fix it. He obtained a crush injury resolving in amputation of the distal phalangee. He was operated on by the hand surgeon who created an advanced flap closure. He was admitted to internal medicine because he had elevated blood pressure and the hand surgeon don't admit patients.

He had a history of hypertension, long-term smoking, and a sedentary lifestyle. On physical examination, bibasilar crackles were noted on lung auscultation.
