9 year old girl fell off around 4 staircases and twisted her foot in a way she doesn't remember exactly how.. swelling and tenderness around the medial aspect of her right foot. What do you see? Whats your diagnosis? And how would you treat?

3 yo F presents to UC with father. He states she was running in the park and fell- since then won't bear weight on affected foot. On exam the foot is swollen and TTP over mid and forefoot (hard to elicit point TTP as child was crying through the exam). No abrasions. Able to wiggle her toes on command but won't bear weight. Ankle and knee move freely without pain. VSS. What do you see and how would you manage if this was your clinic on the weekend?

46 year old female with a history of lupus and chronic steroid use sustains a fall injuring her left ankle. She was seen at an outside hospital where she stated her ankle was reduced. On exam she has robust diffuse swelling. Radiographs and CT images note the trimalleolar fracture with talar translation. She is splinted with strict elevation instructions pending surgery. How would you fix this injury? What approache(s) would you use? What would be your order of fixation?

Ortho suggested surgical repair. How would you manage? What other information is necessary?

Motorcycle crash. Open foot fractures. Reduced. Washed out in ED and patient to OR. Will post follow ups. Ancef and Tdap as well. How would you fix?
