This ECG belongs to a 60 years old male that was seen by me in GP clinic. He is a known smoker 1pack/day, with hx of old PTB (completed treatment).

The chief complaint was lethargy. Prior to presentation he was driving 8 hours to my town for an event. Upon arrival the night before he experienced mild chest pain in which he able to bear and slept throughout the night. The morning after, his family members noted he was so weak that they had to carry him into the car.

He was wheeled-chair into my room, GCS full but evidently lethargic. BP on arrival was 170/90 with pr 50. SPO2 99%, lungs clear, no pedal edema. He was still having chest pain, thus one sublingual GTN was given.

ECG stat is as above.

An ambulance was immediately called and the patient was brought to nearest district hospital. From what I've been told, he was started on fibrinolytic therapy (Streptokinase) and successfully reperfused.
