34yo Male. Multiple previous issues with large outbreaks, however most don't grow to that size. PT's wife has begun removing any blackheads that appear in outbreak areas. Advised loose clothing, wash with abx soap, weight loss, but despite doing all this, there seems to be no relief. Any other thoughts that might help him?

Treated with allergy medication in addition to oral (ivermectin) and topical (permethrin 5%) scabies medication symptoms continue to persist with new outbreaks occurring. Does the skin outbreak look like a typical crusted scabies outbreak? If so would retreatment with a different medication prove to be more successful ? Thank you.
