Post contrast MR image in a 4 year old with a proptotic eye shows a large, enhancing left globe mass with extraocular extension (blue arrow). The initial CT obtained in the ER did not show clear calcification as is frequently seen in retinoblastoma, and this mass was found to be an ocular melanoma (OM). OM is a malignancy arising in the melanocytes of the uveal tract and is much more commonly found in adults. Extraocular extension is a poor prognostic sign, and death usually occurs as a result of distant metastases, such as in this case. #NeuroWednesday
