27yo/m callS 911 for minor chest pain on and off for the last few weeks. States today he’s experienced difficulty breathing. He states he’s had a minor temp for the last couple days. He denies any past medical hx. Patient is calm and in no obvious distress. Skin PWD. Ddx? Ekg?

Dispatched to residence for sick person. Upon arrival patient is a 65yo/m found sitting outside home on steps. Family member states her uncle has not been acting normal over the last 5 days. She states he is lethargic and confused at times. She also states he has fallen around 8x over the last two days and is incontinent with dark foul smelling urine. At current time patient is A/Ox4, skin P/W/D however appears to be in pain. Patient states he has experienced shortness of breath over the last few days with sharp non radiating LUQ abdominal pain. Initial o2 sat was 79% on RA. Auscultation revealed crackles bilaterally in lower lobes. Thoughts? Treatment?
