55-year-old lady presented with haemoptysis accompanied by loss of appetite and weight of 15kg over 4 months. Serial xrays taken over 8 weeks duration showed progressive RUL lung opacity into whole white-out right lung opacities. Unable to tolerate flexible bronchoscopy or CT-guided lung biopsy due to cough and oxygenation. Differential diagnoses apart from lung ca were pulmonary tuberculosis or fungal lung infection. HPE from aspiration of swelling over cervical vertebrae (C2/C3) together with CT neck/lung confirmed lung adenocarcinoma with lung mass extended to cervical area.
