The patient started yesterday with this ulcer, non painful, no secretion, no lymph nodes. Not tender or painful at palpation. Personal sex life: last partner was in jail, had sex without condom, they started dating a month ago. Syph vs HVS2.

Lesions developed over 4 months
Initially perianal and spread to involve perineum and scrotum. Non-painful.
Also developed hard non-tender bilateral inguinal lymphadenopathy.
No ulcers on penis but thickened indurated skin over shaft and
foreskin. Some symptoms of urethral outflow obstruction. Pain with
defecating.
Contact with sex worker in Indonesia prior to onset of symptoms.
No generalized adenopathy. No fevers or weight loss.
HIV and syphilis (-)

27 yo HIV positive male, cd4 600 undetectable viral load, has unprotected anal sex (serosorting) with multiple partners, complains of itchiness and foul smell. My guesses are #Balanoposthitis due to candida? Bacterial? Gave him #Miconazole Cream. For glove police; patients' hands, not mine.

38yo middle-eastern patient presented with 10days lesion near the frenulum of the prepuce, patient is sexually active with recent multiple partners. He complained of non-foul smelling discharge from the lesion associated with itchiness. STI screening with VDRL all negative. He admitted had rough oral sex 2 weeks PTC. Gave him Bactroban ointment and told probably it was from infected abrasions due to rough oral sex trauma. What do you think?
