Case Report

86M w/progressive weakness & diarrhea, +new confusion today. A&Ox3 at baseline. EMS note pt to be bradycardic, altered, with low temperature. Pt received 0.5 mg atropine w/o effect. VS abnml on arrival. PMHx Crohn's, remote DVT on warfarin, anemia, CKD, abnml lytes, anasarca. Recent 2-wk hosp stay for Crohn's flare, followed by 2-wk stay for pneumatosis intestinalis, Tx'd w/Abx & steroids, developed AKI & stable pleural effusions, warfarin stopped. D/C'd to SNF on prednisone taper 5 days ago. Img1 arrival EKG, Imgs2&3 priors from 6 wks/2yrs ago, Img4 CT, Img5 labs. EKG analysis? DDx & Tx?

Dispatched for a male who fell and cut his chin. On arrival, heart rate ~25 with EKG 1 and 2. BP 70/40. Atropine 0.5mg (IV, ASA too) brings him to EKG 2 with a BP 130/80. Unremarkable transport, and sent to cath as soon as it was prepped.

55m pmhx renal insufficiency presenting with 2 wks of cp 1 w/o of left shoulder pain, 2-3 days of dypnea on exertion. Heart rate 128, irregularly irregular. BP 124/82 (other arm was 118/78) Pulses symmetric throughout. EKG showed new onset Afib.

65asian F w 6 wks of recurrent pruritic papules (almost vesicular) on lower leg, arms, back. Husband (a doctor) had similar lesions 1.5 yrs ago and derm recommended allergy tests and now on pollen shots w improvement. Pt will get biopsy if consensus opinion is to get one.

Pt arrived via EMS. Very little history. CC is AMS with n/v. Initial EKG is shown. What do you see? What work-up would you initiate? EKG 2 is after appropriate management.
