Case Presentation

A 48‐year‐old man presented with a 4‐month history of asymptomatic, dome‐shaped, red‐brown, shiny surfaced papules and nodules on his face, limbs and trunk. Imaging, clinical examination and laboratory findings were unremarkable. Histopathology demonstrated numerous infiltrating histiocyte‐engulfed inflammatory cells, forming ‘emperipolesis’. Immunohistochemically, histiocytes were CD68 and S100 positive, but CD1a negative. Cutaneous Rosai–Dorfman disease (CRDD) was diagnosed. CRDD is a rare entity, especially presenting as generalized acneiform lesions. The shiny surface and absence of pustules may be a useful clinical clue to distinguish CRDD from acne, and the histological and immunohistochemical features play a decisive role in diagnosis.

  1. A 21-year-old male patient presented with enlarged submental and posterior cervical lymph nodes persistent for around 6 months. The examination showed two submental lymph nodes around 1cm and one posterior cervical lymph node just under 2cm, with overlying skin appearing normal. History suggested no acute systemic symptoms and previous treatment included antibiotics.

  2. A 9-year-old male patient complained of sudden appearance of tiny shiny papules without other symptoms for the past week, questioning if this could be lichin nitidus.

  3. A child with Rosai-Dorfman disease showcased multiple enlarged cervical lymph nodes noted on MRI.

  4. A 52-year-old man presented with a 20-minute history of chest pain radiating to the left shoulder and neck, alongside dyspnea and diaphoresis. History took note of pre-existing hypertension and lifestyle factors.

  5. A 28-year-old woman presented with intense pruritus over the lower back with multiple small vesicles noted during examination. No recent area exposures were reported.

  6. A 4-month-old male infant has developed skin lesions localized to the chin over the past 3 days with pustules observed.

  7. A patient in late teens collapsed without prior chest pain or significant medical history. Inquiry into the EKG features was noted.
