Case Summary

52 yof, hx GBS. Presented initially (2 wks prior) w/ fever and malaise. Labs showed thrombocytopenia, leukopenia, and coagulopathy, and elevated iron. Bone marrow biopsy: MPD w/myelodysplastic syndrome. 1 wk prior: Developed ascites. Liver biopsy: acute hepatitis, cirrhosis, no iron deposits. + EBV IgG, HSV1 IgG, CMV IgG. Negative cultures, Hep panel, and Lyme. Developed L SDH, transferred to my facility, OR for evac with craniectomy. 2 days later, encephalopathy and seizures. Next day CT showed L frontal ICH and hypodense brainstem.
