52yo female EKG with an L-VAD
Hello I’ve been a APRN for over 1. Primary care is still new to me. Patient is 72 year old complaints of leg swelling. He was due to follow up with cardiologist last week but failed to make appointment. He denies shortness of breath. Past medical history includes CHF, CABG x4, ICD placement 2023, HLD, decreased GFR 70, BPH, Parkinson’s. I wanted to make sure I was headed in the right direction when it comes to his care. I planned on doing EKG in office, ordering BMP and pro BNP, wanted to get him in cardio ASAP. Currently on Lasix 20, Entresto 24-26mg, Tamsulosin, Metoprolol succinate 25, Spironolactone 25. Debating if increasing Lasix from 20 to 40 mg would be effective. Any thoughts or suggestions would be greatly appreciated.

Patient in late teens presenting after a collapse, no chest pain, no previous cardiac history, no history of sudden death in family, are there any features in this EKG that would warrant further work-up or is this just a pediatric EKG?
Early 50s female with sudden onset CP. No significant cardiac hx. SCAD down LM to mLAD. 5x DES w/ good result. Worsening CV shock over the coming days. LVEF 16%. Inotropes and pressors started and eventually weaned completely over a few days.

Her chest X-ray shows central pulmonary vascular congestion. She underwent dialysis yesterday. On physical examination, she has jugular venous distension, bilateral crackles on lung auscultation, and mild peripheral edema. Her blood pressure is 140/90 mmHg, heart rate is 88 bpm, and she is afebrile. Laboratory results are significant for the following: BUN: 5 mg/dL, Creatinine: 7.5 mg/dL, Potassium: 5.8 mEq/L, BNP: 1,200 pg/mL, Sodium: 136 mEq/L, Hemoglobin: 11 g/dL. Is there a condition or diagnosis when enough fluid has not been removed?
40s y/o male PMH Afib w ablation, HFrEF <20%, HTN, DM, obesity. Medications: amio 200, Lisinopril 2.5, Lipitor, Coreg 12.5, Lasix 40. Admitted to different ICU for COPD, HF exacerbation?. CXR bilat infiltrated, no fever, procal negative, lactic 2.1. Azithro and Rocephin started.
Patient developed AFIB RVR, medications were tweaked and TX to our CICU. Upon arrival echo is performed, findings shown in picture. What do you see? What is the course of treatment? If mechanical support is necessary what mechanical support is indicated if any?
