16 yo male. Scar revision for hypertrophic (painful) scar 2yrs after port-a-cath removal for chemo. (Had radiation to brain and spine 3 years prior, may be reason for chest scar issues) Non-healing wound, referred for Hyperbaric Oxygen Therapy (HBOT) x 8 weeks. Last two pictures show result currently. Wound closed, but recurrence of hypertrophied scar. Using silicone strips. Any other suggestions?

tissue loss and necrotic debris, what wound care advice would you give this patient? 6-0 absorbable used, how bad will be the scar?

80+ yr old PT who takes coumadin daily, who fell and had a huge open wound on L hand. Pic 1 was taken 3 weeks ago, pic 2 was 2 weeks and 1 was today. Wound was healing pretty good. Wound tx: Cleanse with saline, pat dry, apply mupirocin 2% ointment, cover with mepitel and dry dressing, secured with kerlix and tape.
