31 to Asian female with prior atopic dermatitis developing rash starting at 2nd trimester and now 19wks pregnant. Started as back plaque of eczema with some response to topical steroid but developed very itchy red bumps (different appearance) on lower legs under breast, around nipple, around umbilicus. Itch is unbearable at around 3Am. Dermatologist prescribed oral prednisone and no relief as of yet. Biopsies pending. Husband has no rash or skin complaints. Dog is receiving flea treatment although no external signs of fleas.
the differential diagnosis could include:
Scabies, which is supported by the severe nocturnal pruritus and linear arrangement of some lesions.
Papular urticaria, due to the presentation of itchy red papules and possible exposure to insect bites or fleas from the pet.
Polymorphic eruption of pregnancy (PEP) or Pruritic Urticarial Papules and Plaques of Pregnancy (PUPPP), considering the pregnancy context, although the typical distribution for PUPPP is the abdomen and often spares the umbilicus.
Eczema, (particularly contact dermatitis) with the presence of crusted and excoriated papules, and a personal history of atopic dermatitis.
Dermatitis herpetiformis could be considered, despite being less common in the patient's demographic, due to the presentation of grouped excoriated papules.
