68 yo aa m presents for annual eye exam. Cc blur od x 1 mo and os x 2-3 years. SCVA OD 20/240 OS 20/400. Pt reports cat sx os 6 mo ago. Obtained notes from cat sx who noted "old rd" OS and referred pt to retina specialist. Pt did not go. Systemic hx: diabetes x 25 years. Patient has a retinal consult scheduled tomorrow.
