Lightning damage. Patient referred for cataract clearance of the left eye about 2 months ago which revealed white cat and hazy/ no view. Bscan clear and patient cleared. Pt returns for eval and after I worked patient up reveled APD and not much improvement in vision. Count fingers at 6 inches to count fingers at two feet. Realized something was missing. Patient reveals being stuck by lighting 4 years ago and as pictures reveal total optic atrophy and chronic CME. Further questions reveal complete rupture of left knee from strike as well. Advice for the day: Always ask patients about recent trauma.

Lightning injuries typically involve multiple systems. This patient presented with a distinct burn after being indirectly struck by lightning. This ferning pattern, known as a Lichtenberg figure, occurs due to the electrical current travelling along the skin's surface before being discharged to the ground. These markings can help indicate to medical examiners the cause of a patient’s death. In survivors, they generally fade within hours or days.

60 years old male pedestrian vs bike presented to ED in drowsy mentality. Biker was speeding through the pedestrian crossing. Pupils - Right 4mm RTL, Left 7mm NRTL, GCS - E3V5M5, CNS - Drowsy, arousable, but unable to count fingers or see anything in the left visual field.
24 year old with choroidal melanoma. This is post plaque radiation and a single Avastin injection. You can see the scarring and mass in the upper left corner of the picture. Patient has count finger vision but the tumor has decreased in size and is currently stable.
