Went to pick up a stemi transfer from a small community hospital. 58yo male only complaint was slight dizziness and diaphoresis over the past few days. 12-lead showed inferior wall mi. Pt a&ox3 with no complaints during transport. He was in better spirits than his daughter who was nauseous the whole way! BP 108/66 HR between 70-80 spo2 98% on 2lpm. Pads were placed for the transport but the patient was so stable and mentating so well I didn't bother. The cath lab seemed completely unconcerned with the rhythm.

84 y/o Male with a recent (within 2 week) cath for a STEMI. Complaining of acute intermittent palpitations and generalized weakness. Rhythm during transport consisted of mostly ectopic beats (PVC’s, PAC’s, PJC’s). Significant right axis deviation was found and during transport, pt began to have couplets, triplets, and runs of PVC’s. Treated with a 150mg Amiodarone drip over 10 minutes during transport. Patient, who was a retired cardiologist, refused transport unless cardioverted on scene. So the doctor in charge decided for sedation and cardioversion. These are the post-CV ECGs, chest pain persisted, patient then accepted transport. He received amiodarone and was transported by ambulance to the nearest facility.
