Case Summary:
Distal femur fracture in 12 yo M.
PT was out with a church group (Hispanic/Latino) at a local indoor trampoline "fun house" dispatched to a low priority fall. Pt found laying on the floor ( which was all stretchy/springy canvas material used in trampolines). Initial assessment revealed point tenderness, edema and deformity assumed to be proximal tib/fib. All other VS stable PT handles the pain very well until we began stabilizing with bulky "blanket splints" Administered 50 mcg fentanyl IN, continued with a very bouncy extrication. Once in ambulance parents were contacted. Monitored VS, I initiated IV, re stabilized, monitored PMS distal to injury. 50 more mcg upon arrival at hospital after 20 min. transport to help with pain while moving off of stretcher. PT was flown to nearest pedi trauma center because none of our surgeons would operate. Poor little guy asked me if he was going to miss school Monday because he had a field trip he wanted to go to.
