76 y.o. M presented to the ED with “ankle pain”. Had been drinking with friends the night before and tripped but friends took him home and he slept through the night. Woke up the next AM and attempted to walk but “noticed his ankle was crooked” and couldn’t walk so called EMS. Stated only 2/10 pain in ankle. Reduction performed in ED before transfer to Trauma Center due to hemoperitoneum from subcapsular renal hematoma.

35 yr old male slipped while walking the dog. Injured his ankle and cane to the ED. Teaching points: pain at lateral mal, unable to bear weight more than 4 steps (Ottawa Ankle rules). AP view shows shortened fibula (Dime Sign), mortise view shows ankle incongruity (Shentons line) and increase medial gutter (>4mm).
