38y.o Female with black stool and fatigue

A 38-year-old female presented with a chief complaint of black stool and fatigue for more than a week prior to admission. Her complete blood count revealed mild hypochromic microcytic anemia, a normal leukocyte count, and thrombocytosis. A fecal occult blood test (FOBT) was negative.
