Teaching point
Mature ovarian teratomas are composed of elements from multiple germ layers and commonly contain macroscopic fat and calcification. Fat density within an ovarian mass, especially when combined with coarse or tooth-like calcifications, strongly favors this diagnosis. In pediatric and adolescent patients, the mass effect of a teratoma increases susceptibility to adnexal twisting.

33 yo G4P4 underwent laparoscopic bilateral Salpingectomy for sterilization. Incidental finding of ~ 6cm Ovarian Dermoid. Pics S/P RSO. pt doing well. Path showed benign Dermoid cyst. First pic arrow shows thick, fatty contents of cyst. Second pic shows orange arrow pointing at hair ball. Blue arrow shows piece of bone.

#Uterus , #Cervix , bilateral #Fallopian-tubes , bilateral #Ovary(ies), and portion of sigmoid #Colon, sent for invasive ovarian #Adenocarcinoma. The ovarian mass invades into the adherent portion of bowel, which is why it was removed. Orange arrow points to uterus/cervix, blue arrow points to portion of sigmoid colon, yellow arrow points to ovarian mass, and the green arrow points to the uninvolved right Fallopian tube and right ovary. The opposing side of the specimen shows the ovarian mass breaking through the outer surface of the ovary, which isn't visible here.
