A 42‐year‐old immunocompetent woman presented with a 1‐year history of a slowly growing mass on her left foot (a). Histopathology showed clumps of filamentous organisms and grains, the latter surrounded by matrix (b). Biopsy tissue culture grew Gram‐positive Gordonia (also known as Gordona terrae) (c, d), which was confirmed by 16S rRNA sequencing. Magnetic resonance imaging demonstrated cellulitis without involving the bone (e). A diagnosis of actinomycetoma was made. The lesion was resolved after being treated with amoxicillin and clavulanic acid for 3 months (f). Mycetomas, commonly occurring on the foot, are chronic, progressively destructive, granulomatous infections caused by fungi (eumycetoma) or bacteria (actinomycetoma). G. terrae is ubiquitous in the environment and often isolated from soil and water. Actinomycetoma caused by G. terrae is very rarely reported. G. terrae can produce pale grains in cutaneous tissue. Gordonia, Nocardia, Rhodococcus and Mycobacterium can easily be misidentified on smear microscopy because they are all actinomycetes. 61yo M with rash starting on foot in summer. Initially responded to methylprednisone and PO steroid taper starting at 50mg and doxycycline. Rash recurred with diffuse spread and improved some to repeat PO steroid taper and doxy. ANA panel, TSH, CRP, ESR negative/normal after initial flare. KOH prep negative in office in early fall and repeat positive for hyphae started bleach bath, outpatient. Unable to pick up terbinafine. Returns today with worse rash, fissures, pain. T2DM otherwise no history autoimmune disorders. He is adopted. 49 yo man with ulcer in his left heel for a month. It began with pain and blister, then it erupted and became ulcers. He has no history of diabetes, but had been a heavy smoker (around 40 cigarettes a day) for 30 years. There is no coldness in his foot, no claudication, good pulse. Non fasting Blood glucose 198. I treated this as buerger, i gave him antibiotics, painkiller, debridement, and gave him cilostazol, hoping to improve the blood flow. The patient then stop his smoking, and after 2 days of smoking cessation, the pain increased so i gave him codeine to relieve his symptoms. I dont have doppler.
