Neurology Case Report
Patient Profile
- Demographics: 68M, African-American, retired Navy
- Presenting Issues: Events of confusion and agitation, frequent headaches, sleep disturbances.
- Family Observations: Wife reports patient displays "weird" breathing patterns while asleep, described as loud snoring.
- Symptoms Onset: 2-3 months ago; patient perceives recent memory issues.
Medical History
- Cranial Examination:
- Complex sentence structure, good vocabulary.
- Not oriented to the year.
- No cranial deficits observed.
- Subtle left arm dysdiadochokinesia and left leg dysmetria noted.
- No hyper-reflexia observed.
Diagnostic Imaging
- CT Head: Significant for severe cerebellar atrophy.
- MRI Brain: Showed cerebellar atrophy without enhancement or restricted diffusion.
- My assessment detected restricted diffusion around the 4th ventricle and T2 hyperintensity.
Recent Procedures
- Lumbar Puncture: Conducted today.
