women in her 50s known case of uncontrolled diabetes, hypertension and dyslipidemia complaining of a 1 year bilateral painful leg swelling with erythema, and ulceration. Loss of sensation in both legs, can’t walk. Couldn’t obtain more details. I’m a medical student and I wish you’d help me understand what’s her diagnosis? Thank you

I saw this case in my Dermatology rotation today. A 14 years old diabetic girl who came with her mother to the OPD as a follow up complaining that the lesions over her both legs which started to expand five years ago until she started the medications but still Not improving. No family history of the same condition and her labs showed that her diabetes is uncontrolled. Necrobiosis lipoidica is a disorder of collagen degeneration with a granulomatous response, thickening of blood vessel walls, and fat deposition.
