42 year old male presents to the local urgent care with atypical chest pain. PT states he has occasional chest numbness, bilateral arm pain and tingling and upper back tightness. Bilateral BPs equal at 186/102 after 2 nitro. First two ECGs are done prehospitally at approximately 40 minutes after onset of discomfort, third is approximately an hour to an hour fifteen after onset. PT is a airline pilot that has come to our city after back to back three hour flights and has been to Mexico within the last month. No PMH no meds. He denies recent nausea, vomiting, diarrhea, and malaise. Family history of sudden cardiac related death in their 60s. Is a tobacco user.
Unfortunately I'm prehospital, no labs or Chest x-ray available.
