A man in his forties, with a history of liver transplantation (recipient CMV IgG positive) performed 4 months prior in India for end-stage liver disease secondary to ethanol abuse, presented in the US with fever, chills, night sweats, malaise, as well as right upper quadrant and epigastric pain. Image shows contrast and non-contrast CT abdomen.

T2 (image 1) and T1-hepatocyte phase post-contrast MRI of the liver in an adolescent with autoimmune hepatitis shows a markedly abnormal appearance of the liver. On the T2 image, there are lace-like areas of increased signal. On the post-contrast image, the liver should have a homogeneous bright appearance. Instead, all of the dark areas are abnormal. The pattern is typical of hepatic fibrosis. Autoimmune hepatitis is often a diagnosis of exclusion. Patients have elevated serum alanine aminotransferase and IgG.
