80 year old female presents to the ED complaining of right sided rib pain radiating to the back. She states this pain began last night. She states it’s made worse on inspiration. She is A/ox3 with skin PWD. She appears in no obvious distress. Lung sounds are slightly diminished on the right side. She had a hiatal hernia repair 1 week ago. STEMI alert is called and the catch lab is activated. Cardiac echo shows suspicion for possible posterior infarct. Patient is taken to the cath lab and results are pending.

59 Yom cc of chest pain. patient has no major medical history. stated that there had been chest pain x3 days but this pain was different. stated this onset of pain started approximately 4 am. patient placed on the monitor and saw elevation right off the get go (I'm a medic student so I'm still learning all of this).
