UPDATE Here is what we just found in our 40yo runner. Aspiration wasn't possible due to massive calcifications. Some "calcium milk" was found inside as well. And the patient's blood phosphate is 6.75 mg/dL (normal range being 2.4 - 4.1 mg/dL). So which #rare diagnosis is confirmed ? (congrats @imsogreen !)

A 45 years young male presented with vertigo and weakness with fever and cough for last 15 days unresponsive to conventional antibiotics. His ESR was 89 mm in 1st hour, Hb was 8gm/dl and S. creatinine was found 2.5 mg/dl. What further investigation we need to do confirm the diagnosis after observing the Xray skull?
