Non-healing wound

75-year-old male with non-healing lower leg wound. Advised that it was pyoderma gangrenosum, but it is not responding to treatment. We are very rural and out of town wound care is not an option due to location and transportation. 2+ pitting edema in the foot. Biopsy stated “inflammation”.

78-year-old female presented to PCP with a heel wound that was recently evaluated by podiatry and was told to put vaseline on it. Second photo is after basic debridement in office. There is no wound care in our rural area and patient is unable to travel. I believe this needs further sharp debridement. Thoughts?
