A 41 year old otherwise healthy man comes to see you with right ankle pain. He was playing soccer, jumped for the ball, landed on both feet and then suddenly felt a pop and sharp pain in his right ankle. On exam he has mild swelling over the posterior aspect of his ankle, his foot is largely dorsiflexed and he has difficulty with plantar flexion. He meets none of the Ottawa ankle/foot criteria. When his calf is squeezed, there is no corresponding plantar flexion. What is the name for the diagnostic physical exam maneuver performed here? What might you expect to see on ultrasound? Finally, what's the diagnosis and how would you manage this patient?](https://app.figure1.com/case-detail/48de4933-52be-4457-bc6f-b2031876f769)

40 year old male sustains a right mid substance Achilles tendon rupture playing flag football.
He has an Achilles tendon repair through a mini-open approach with the pars speed bridge system 3 days after the injury. Case went well. He was in splint for 2 weeks postop and doing well. He was then placed in a cam boot with heel lifts and began rehab.
At 4 weeks postop, he presented with increased pain, malaise, purulent wound drainage, swelling, and redness. (Images)
He is placed on the surgical schedule for debridement and irrigation.
Would you remove all of the sutures of the repair?
Would you remove the swivelocks in the calcaneus?
Would add anything else?
What would your rehab look like postop and depend on what factors?](https://app.figure1.com/case-detail/6d9a4d5d-2aaf-455b-948a-103ead70b6f4)
