39-year-old male patient
Chief Complaint: Right inguinal pain and discomfort.
History: Patient presented with progressive right inguinal discomfort, worsened by physical activity. No gastrointestinal symptoms.
Diagnostic Workup: Ultrasound showed a right inguinal canal defect (~18.7 mm) with protrusion of intra-abdominal fat forming a hernia sac (2.9 × 1.5 × 1.9 cm; ~4.6 cc). The hernia was fully reducible, with no bowel involvement or signs of incarceration.
Diagnosis: Right reducible inguinal hernia.
Treatment Plan: Laparoscopic TAPP repair with mesh placement.
Recurrent complex incisional hernia
Chief Complaint: Midline abdominal bulge and discomfort.
History: Patient with a history of seven prior failed hernia repairs presented with a recurrent midline incisional hernia.
Diagnostic Workup: Abdominal CT scan revealed a ~6 cm midline incisional hernia.
Diagnosis: Recurrent complex incisional hernia.
Treatment: Abdominal wall reconstruction with left-sided hemi-transversus abdominis release, primary midline closure, and retromuscular polypropylene mesh placement.
Outcome: Uneventful postoperative course.
