60yrs old female having accidental head injury brought to ED in unconscious state with decerebrate rigidity, immediate intubated then put on venti support, Exact mechanism of injury unknown. No penetrating and blunt injuries present. O/e CNS-Unconscious, Pupils - Ipsilateral mid dilated, GCS-E1M2V1, Bp-170/90, Pr-48/mt. After initial resuscitation and stabilisation a CT scan of head was done. Diagnosis and management plan?
