Diafragmatic traumatic hernia
Case Description
- Patient: A 39-year-old female presented with a noticeable bulge at the level of the umbilical scar, which had progressively increased in size over the past months.
- Symptoms: The patient reported discomfort exacerbated by physical effort, prolonged standing, and activities that increase intra-abdominal pressure.
- Physical Examination: On inspection and palpation, a reducible umbilical protrusion was identified at the site of the umbilical scar. The defect was more evident with Valsalva maneuver.
- Diagnostic Workup: Clinical findings were consistent with an umbilical hernia. An abdominal wall ultrasound was performed for diagnostic confirmation, revealing a fascial defect at the umbilical level with herniation of preperitoneal fat, without evidence of incarceration or bowel compromise.
- Treatment: An open umbilical hernia repair was performed with placement of a 7.5 x 7.5 cm polypropylene mesh in the preperitoneal plane. The mesh was positioned to ensure adequate overlap of the defect margins, providing tension-free reinforcement of the abdominal wall.
